In order to work at any grade Doctors in the UK you must be Registered with the GMC with a License to Practice and have Proof of Right to Work in the UK.

Junior Clinical Fellow
A Junior Clinical Fellow, also referred to as JCF, is a qualified Doctor in a non-training role. Whilst you will have been exposed to cases and have the ability to triage before seeking advice, you will be working under the supervision of a more senior colleague who is there to support, guide and advise you.
Senior Clinical Fellow
Also referred to as SCF, this grade is similar to that of a Senior SHO / Junior SpR and works at the equivalent level as a higher ST4+.
Clinical Research Fellow
These roles are usually offered on a 12-month contract and can be a good route to start you on the road to obtaining your MD, should you wish to do so. You need to be willing and eager to take on the challenge of a research project alongside your clinical duties. Your clinical duties will of course involve being assigned to clinics along with specific clinical duties, however, they will be fewer than those expected of other clinical colleagues.

Predominantly permanent posts for experienced Doctors who have four years full time post graduate experience of which two are in the relevant specialty. The majority of a Specialty Doctors’ time is focused around patient care.
Specialist Grade
A new post here in Wales which has recently been successfully launched. The Specialist Grade is a very senior autonomous role which sits in between Specialty Doctors and Consultants. To be eligible for this role you will need to be twelve years’ full time equivalent postgraduate and six of those must be in the relevant specialty.
Locum Consultant
The Locum Consultant role is predominantly for covering short term vacancies within the service. According to Royal College Guidance Locum Consultant contracts should be no longer than 12 months in total, 6 months initially and then reviewed and extended a further 6 months if required.
Substantive Consultant
Consultants can only be appointed through a Regulated process called an Advisory Appointments Committee (AAC) and the NHS Appointment of Consultant Regulations 1996 (Statutory Instrument no 1996/701) set out the rules for this.
